29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Carbo Forge, Inc., located at 150 State Route 523, Fremont, OH 43420: On or about June 24, 2019, the employer did not ensure that one or more methods of adequate machine guarding were provided to protect employees from hazards such as, but not limited to, the pinch point created between a bar and the rollers on the 530 Ton Hydraulic Shear (Advanced Machine Design Company, Model # 530-2230; Serial # 9102) located in the Shear Building. Employees were exposed to pinch point hazards that could result in amputations.
