29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about April 29, 2019, the employer failed to ensure that a maintenance employee at the facility who was operating various powered industrial vehicles was trained and authorized to operate the vehicles. The employee who was not trained operated the Mitsubishi, FGC25, serial #AF8F31682.
