29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: a) On or about May 30, 2019, employees working just east of the Number 2, Cincinnati Milacron Molding Machine were exposed to an energized 120 volt Dayton floor fan. The fan's attachment plug was missing a ground pin. The employees using this equipment were thereby exposed to an electrical hazard.
