29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) On or about March 25, 2019, an employee was hospitalized after being found on the ground from an apparent loss of consciousness and fall to a lower level. The employer failed to report the hospitalization within 24 hours and OSHA became aware of the hospitalization on or about May 21, 2019.
