29 CFR 1910.38(f)(3): The employer did not review the emergency action plan with each employee covered by the plan when the plan was changed: Location: 475 Edgartown Rd, Vineyard Haven, MA 02568 On or about May 7th, 2019 employees were exposed to caught between, contact with fire, and inhalation hazards while working where the emergency evacuation plan was not updated, posted, and the employees were not trained on the new plan after the building was expanded and reconfigured
