29 CFR 1910.216(b)(1): An accessible safety trip control that operated on contact was not provided in front and/or back of each mill. Note: The safety trip control shall be one of the types or a combination thereof as listed in 1910.216(b)(1): On or about 4/29/2019, the Cracker Mill located in the Refining Department was inadequately guarded. Although the mill was operated in combination with a feed conveyor; the safety trip located at the rear of the mill was not properly adjusted across the bite and was not used in conjunction with a fixed guard bar or pressure sensitive body bar. Operators and maintenance employees tasked with operating and/or servicing and maintaining the mill were not protected from hazards created by the ingoing nip/rollers.
