29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: Naval OLF NEX: On April 23, 2019, employee(s) were repairing a wireless pressurized line leak detector (WPLLD) in a fuel sump while the equipment was energized. The WPLLD broke and fuel sprayed from the equipment. Employee(s) manually disengaged the yolk assembly to isolate the energy, which ignited the fuel vapors, and created a flash fire. Training on the energy control program was not provided prior to authorized employees performed maintenance/servicing on equipment, such as the fuel sump system. Employees were exposed to fire and/or explosion hazards.
