29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: Outside facility - On or about April 6, 2019 an employee was operator a Toyota sit-down type LP-gas model 7FGU45, SN # 61243 forklift exposing employees to struck by hazards in that employee was involved in a forklift accident on April 6, 2019 which resulted in an employee been injured and had not been retrained.
