29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system: The employer is failing to protect employees from crushing hazards while working in a trench. The most recent occurrence was evident during the inspection conducted on April 9, 2019 at a worksite located at 410 E 8th St., Stromsburg, NE 68666. The employer allowed an employee to enter the trench, which is about 50 feet long, 7 feet wide, and 9 feet deep, even though it was not secured by a protective system. As a result an employee was injured when the trench wall caved-in while he was in it looking for the city sewer stub.
