29 CFR 1926.1053(b)(4): Use. The following requirements apply to the use of all ladders, including job-made ladders, except as otherwise indicated: Ladders shall be used only for the purpose for which they were designed. On or about April 5, 2019, the employer failed to ensure employees were using the ladders provided at the job site as they were intended to be used. An employee was observed, to include but not limited to, standing on the 2nd rung from the top of a 15 foot ladder and while holding on, was trying to "hop" the ladder over on the roof to access another section of the roof.
