On or about April 4, 2019, in the extrusion department, the light switch for the light fixture above the operator's work area for the 1675 Wean United Youngstown Foundry & Machine aluminum extrusion press had an opening in the left side due to a knockout that was missing, thereby exposing employees to an electrical hazard.
