29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling: On or about March 8, 2019, the employer provided listed/labeled equipment that was not installed and used in accordance with the instructions and employees were exposed the following: a) fire hazards from a relocatable power tap used to power a refrigerator (115V/6A), microwave (120V/9A), and coffee maker (110V/13A) in the Housekeeping Office; and b) electrical shock and burn hazards from a 6-way outlet adapter that was used to power a register and video machine at the Bar.
