29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about February 28, 2019, the employer failed to ensure defective powered industrial vehicles were removed from service prior to operation of them. A yellow Clark vehicle, serial #356-815-4376, that was being operated had a light out and also had an unsecured propane tank.
