Text For Citation: 01 Item/Group: 004 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about February 12, 2019, the employer did not ensure that employees were protected from pinch point hazards when the #2 pneumatic foam impactor closed. An employee sustained finger amputation injuries when caught between a platen of the impactor and a platen spacer. b. On or about February 12, 2019, the employer did not ensure that employees were protected from pinch point hazards created when the #1 pneumatic foam impactor closed. Employees were exposed to the pinch point created between the machine's platen and spacer. c. On or about February 11, 2019, the employer did not ensure that rotating bits located on the TD-95 router and drill machine were guarded to protect employees against rotating part hazards. Employees that operated the equipment were exposed to amputation and laceration injuries from the unguarded rotating parts.
