29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): (a) At a jobsite located at 3301 South State Highway FF, Springfield, Missouri: Employees were exposed to an unexpected energization or release of stored energy, or recirculation of material from the tank. On or about March 8, 2019, the Digester #3 Recirculation Discharge valve did not have a tag identifying the employee applying the lockout device. (b) Employees were exposed to an unexpected energization or release of stored energy or recirculation of material from the tank. On or about March 8, 2019, the Digester #3 Recirculation Suction or Withdraw pipe was locked out but did not have a tag identifying the employee applying the lockout device.
