29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a- 135 Garden Hills, Lakewood NJ: Company worksite workers were exposed to fall hazards, in excess of 24 feet to the ground surface below, when using a portable aluminum extension ladder that contained visibly damaged rungs, was unsecured, set on an unstable surface, and whose side rails did not extend at least 3 feet above the upper landing surface being accessed. Condition observed on or about 01/30/19. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
