29 CFR 1910.30(a)(3): In that an employee had not been trained in the nature and recognition of fall hazards in the work area; the procedures to minimize those fall hazards; the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; the correct use of personal fall protection systems and equipment. a) Building Interior-Warehouse: The employer did not ensure that an employee was trained to recognize the hazards related to using and installing an extension ladder to retrieve a box of products that was stored on the top shelf of a storage rack. On 1/16/2019, an employee sustained fractures to both of her heels when she fell from an extension ladder. The employee was not trained in the nature and recognition of fall hazards while using a ladder.
