29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about January 10, 2019, employee(s) were exposed to crushing/pinch point hazards and amputation injury while working on and around the Unitrol Solution Welder (Machine BW1, Model #9280D-602 & Serial #S0400508). The point of operation was not properly guarded. (b) On or about January 15, 2019, employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the TAM70 machine. The point of operation was not properly guarded and the interlock was by-passed. (c) On or about January 15, 2019, employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the BAM40 machine. The point of operation and ingoing nip points were not properly guarded. (d) On or about January 15, 2019, employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the BAM20 machine. The point of operation and ingoing nip points were not properly guarded.
