1910.147(c)(6)(i) The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) Stamping Department- On or about March 14, 2019, employees were exposed to machine hazards associated with the unexpected energization of Press #13, Press #14, Press #24 and Press #3673, and the Minster Press, at the facility. The employer did not conduct periodic inspection at least annually to ensure that the procedure and requirements of this standard were being followed for this equipment. b) Bending Department- On or about April 23, 2019, employees were exposed to machine hazards associated with the unexpected energization of Press Brake #17 at the facility. The employer did not conduct periodic inspection at least annually to ensure that the procedure and requirements of this standard were being followed for this equipment. c) Shear Department- On or about April 16, 2019, an employee was exposed to machine hazards associated with the unexpected energization of Shear #1. The employer did not conduct periodic inspection at least annually to ensure that the procedure and requirements of this standard were being followed for this equipment. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
