29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about 11/8/2018 at machinery including but not limited to the Travel Crete machine workers were exposed to injuries related to unexpected release of energies where the specific procedures were not inspected for completeness and accuracy on an annual basis. - Travel Crete machine was identified as deficient where there were no steps in writing for the motor to be locked out to prevent travel during maintenance or troubleshooting activities.
