29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a. Banbury Mixer No. 3 - The employees were exposed to a crushing and amputation hazard. On or about October 31, 2018 the Banbury mixer operator attempted to clean rubber from the discharge door without locking out the mixer and inserting/tightening the safety bolts into the top of the discharge door.
