29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed. On 9/26/18, at the Covington facility, the employer did not conduct a periodic inspection of the Lockout program on an at least annual basis. Deficiencies in the procedures included not including all energies or the magnitudes of those energies and did not include all Authorized employees.
