29 CFR 1910.422(b)(3): The employer did not provide means to assist an injured diver from the water or into a bell: Pacific Ocean off the coast of the Silver Strand, 32.39.3962N/117.09.6871 (LAT/LONG): On August 22, 2018, employees were performing SCUBA diving operations to mark a sunken road line. A diver suffered fatal injuries after he lost consciousness underwater and the dive team was unsuccessful at manually removing the employee into the boat by hand.
