Text For Citation: 01 Item/Group: 004 Hazard:
29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): On or about August 1, 2018, the employer failed to ensure that an employee who was working on the electrical components of Line #11 had applied lock out tag out prior to performing the work. The electrical energy was not fully isolated prior to performing the work and an employee sustained 2nd and 3rd degree burns to his hands due to an arc flash fire.
