29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressing at least steps for each operating phase, safe operating limits, safety and health considerations, and safety systems and their functions as outlined by this paragraph: Facility: Tank Farm, Gas House The company has not developed and implemented written operating procedures for the Tank Farm, Gas House or Reactor Room. Instance A: The company has three different operating procedures for unloading the Tank Farm. The Aeropres procedure that it identified as the procedure governing the unloading of propellants calls for taking a sample and having it analyzed before unloading. This is not done. Instance B: Tank Farm employees do not inspect Tank Farm hoses. Instance C: A Gas House employee was unfamiliar with a document that contained Gas House operating procedures. Instance D: Gas House operating procedures in a Gunderson Systems 2003 document are not up-to-date.
