Text For Citation: 01 Item/Group: 004 Hazard:
29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and shall be done in the following sequence. a. Viking Paper Corporation located in Toledo, Ohio: On or about June 12, 2018 the employer did not ensure an employee properly locked out the cut to size knife before unjamming the equipment. The employee did not shut off the equipment using the electrical disconnect but rather locked out the access door to prevent the interlock from making contact.
