29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential style roof. This was most recently documented at 904 Deer Run Lane, Papillion, Nebraska, on or about May 29, 2018, when the employer had employees access the residential roof with a ladder and perform re-roofing activities. Employees were not trained to recognize neither the hazards of ladders nor the proper procedures to be followed to minimize these hazards. Workers were observed climbing the portable extension ladder, which was set up incorrectly. The ladder stood less than three feet above the upper landing surface (roof) of the building and was not secured to prevent displacement. Workers were exposed to falls of over 11 feet to dirt and concrete below.
