29 CFR 1910.147(f)(4): During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming: a) On or about May 23, 2018 and times prior shift change lockout/tagout procedures: The employer failed to ensure the continuity of the lockout/tagout procedure was maintained during shift change when there was no lock placed Mold machine B-2 between 3rd and 1st shift which had the cylinder removed.
