29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about May 29, 2018 the employer failed to ensure that employees performing service on the 500 Pacific mechanical power press located and affixed a lockout device to both the main control switch and the pneumatic line prior to performing the work.
