29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about May 4, 2018: a) At the manufacturing facility located at 15 Enterprise Way Dahlonega GA, employees performing maintenance activities were exposed to release of hazardous energies, when the employer failed to perform a periodic inspection of the company's energy control procedure. Deficiencies in the specific procedures found would have included but are not limited to the following: a) Haas SL 20 CNC - Did not contain a specific statement on the intended use of the procedure. b) Haas SL 20 CNC - Missing the shutting down and isolation of pneumatic or hydraulic energies. c) Haas SL 20 CNC - Missing the magnitude of each energy source to include but not limited to electrical, pneumatic and hydraulic energies. d) Haas SL 20 CNC - Specific instructions to replace any and all guards removed while performing servicing and or maintenance tasks. e) Fadal VMC 4020 CNC - Did not contain a specific statement on the intended use of the procedure. f) Fadal VMC 4020 CNC - Missing the shutting down and isolation of pneumatic or hydraulic energies. g) Fadal VMC 4020 CNC - Missing the magnitude of each energy source to include but not limited to electrical, pneumatic and hydraulic energies. h) Fadal VMC 4020 CNC - Specific instructions to replace any and all guards removed while performing servicing and or maintenance tasks. i) Okuma Lathe - Did not contain a specific statement on the intended use of the procedure. j) Okuma Lathe - Missing the shutting down and isolation of pneumatic or hydraulic energies. k) Okuma Lathe - Missing the magnitude of each energy source to include but not limited to electrical, pneumatic and hydraulic energies. l) Okuma Lathe - Specific instructions to replace any and all guards removed while performing servicing and or maintenance tasks. m) Green Hob - Did not contain a specific statement on the intended use of the procedure. n) Green Hob - Missing the magnitude of each energy source to include but not limited to electrical, pneumatic and hydraulic energies. o) Green Hob - Specific instructions to replace any and all guards removed while performing servicing and or maintenance tasks. p) Long Hob - Did not contain a specific statement on the intended use of the procedure. q) Long Hob - Missing the magnitude of each energy source to include but not limited to electrical, pneumatic and hydraulic energies. r) Long Hob - Specific instructions to replace any and all guards removed while performing servicing and or maintenance tasks. s) OMAX 80160 Water Jet Table - No specific procedures developed.
