29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: The employer is failing to protect their employees in the recognition and avoidance of unsafe conditions. This was most recently observed at a job site located at: Jaxson Estates, Tennis Court, Lot 284, Wentzville, MO 63390, on or about 4/23/2018. Employees who operate the Schwing Concrete Pump Truck were not trained to recognize crushing and amputation hazards associated with unguarded equipment during cleanout procedures.
