29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about April 9, 2018, and at times thereto, employees were performing service and maintenance activities on the shredder machine. The employer's developed lockout/tagout procedure did not specifically inform employees performing the covered activities which electrical disconnect(s) were to be turned off, include steps to allow drive motors to coast to a stop, include steps for isolating and controlling stored hydraulic energy, and include specific steps to verify the energy sources were at a zero energy state.
