29 CFR 1910.217(c)(1)(i): The employer did not provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. On or about April 8, 2018, the employer failed to ensure that the Reliance 45 mechanical power press was effectively guarded to prevent employees from reaching the point of operation while in use. The press contained a barrier guard with an opening of 3 3/4 inches.
