29 CFR 1910.269(d)(6)(vi): When there was a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed or until the possibility of such accumulation no longer exists: (a) On or about 3/16/2018 - Cooling Tower Lift Pump Station 6.9 KV Cooling Tower Board B Compartment 2, the employer adversely affected the protective grounds associated with HO 0-202-0594 when he disconnected and re-connected ground truck cable phases for continuity checks and allowed for the possibility of 6.9 KV energy re-accumulation hazards.
