29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about 4/6/2018, Employee risked serious injury or amputation while clearing jam for PATZ Barn Cleaner 2071 and waste belt 2072 without locking out equipment. b) On or about 4/5/2018, Employee entered chained off area without locking out equipment to help clear a jam on the West Plains Resaw System risking amputation or other severe injury.
