29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: PLATING LINE: On or about and at times prior to March 21, 2018, by not turning off the rectifier before trying to separate two racks of plumbing parts that were above Tank 15 (Chrome Plate) in the Plater #2 Plating Line, the company operating procedures were not followed. In addition, the procedure was inadequate in that the rectifier was not required to be locked out prior to attempting to separate the racks.
