29 CFR 1910.140(c)(20): Ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection must be protected from being cut, abraded, melted, or otherwise damaged. a) On January 9, 2018 located at 3745 Beechmont Ct. Cincinnati, OH 45226 (Jacob Brothers Heating & Air Conditioning) employees performed sign installation associated operations on upper and lower back unprotected side or edge of Bulletin sign #5502 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 25 feet to the ground below. b) On January 9, 2016 located at 3745 Beechmont Ct. Cincinnati, OH 45226 (Jacob Brothers Heating & Air Conditioning) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #5502 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 25 feet to the ground below. c) On January 22, 2018 located at 3745 Beechmont Ct. (Jacob Brothers Heating & Air Conditioning) employees performed sign installation Cincinnati, OH 45226 associated operations on lower front unprotected side or edge of Poster sign #5501 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 25 feet to the ground below. d) On March 6, 2018 located across street from 4597 Kellogg Ave. Cincinnati, OH 45226 (Cincinnati Interiors, Ltd) an employee performed sign installation associated operations on lower front unprotected side or edge of Junior Poster sign #5577 stage/platform utilizing personal protective fall arrest lanyard connected to a shepherds hook which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 8 feet to the ground below. e) On March 6, 2018 located at 1814 Powers Ct. Cincinnati, Oh 45223 (WAE Church) employees performed sign installation associated operations on upper back unprotected side or edge of Bulletin sign #1192 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 13 feet to the building roof top below. f) On March 6, 2018 located at 1814 Powers Ct. Cincinnati, Oh 45223 (WAE Church) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #1192 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 13 feet to the ground below. g) On January 3, 2018 located at 919 W North Bend Rd. Cincinnati, Oh 45224 (McDonalds) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #3338 stage/platform utilizing personal protective fall arrest equipment which was not used in a reliable condition in that fall arrest equipment system allowed employee(s) cross arm strap and retractable lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 11 feet to the ground below or 22 inches into barb wire fence. h) On January 30, 2018 located in the back of a parking lot between 2325 & 2335 W. Mc Micken, Cincinnati OH 45214 employees performed upper back sign installation associated operations on upper back unprotected side or edge of Bulletin sign #1312 stage/platform utilizing personal protective fall arrest lanyard connected to a wire rope mid rail / horizontal lifeline in which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 15 feet to the ground below. i) On January 12, 2018 located adjacent to old Norwood incinerator at 3001 Duck Creek Road Norwood Oh 45212 employees performed upper back sign installation associated operations on upper back unprotected side or edge of Bulletin sign #5028 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 20 feet to the ground below. j) On January 12, 2018 located adjacent to old Norwood incinerator at 3001 Duck Creek Road Norwood Oh 45212 employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #5028 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 9 feet to the ground below. k) On January 5, 2018 located beside 1400 Reading Rd. Cincinnati, Oh 45202 (Staples) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #1407 stage/platform utilizing personal protective fall arrest equipment connected to a horizontal lifeline in which the lanyard utilized was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 12 feet to the ground below. l) On January 5, 2018 located beside 1400 Reading Rd. Cincinnati, Oh 45202 (Staples) employees performed sign installation associated operations on upper back unprotected side or edge of Bulletin sign #1407 stage/platform utilizing personal protective fall arrest equipment connected to a horizontal lifeline in which the lanyard utilized was not used in a reliable condition in that fall arrest equipment systems allowed employee(s) lanyard to free/swing fall against the edge of the platform and was not protected from sheering/abrasion exposing employees to falls in excess of 16 feet to the ground below.
