29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping such materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. The employer is failing to protect employees from struck-by and engulfment hazards associated with trenching and excavations. This was most recently documented on or about January 30, 2018 at a jobsite located at or near 16 Hawk Ridge Circle, Lake Saint Louis, MO 63367. The employer did not protect employees from struck-by and engulfment hazards by placing or keeping excavated or other materials at least 2 feet from the edge of the excavation.
