29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) Employees engaged in installing concrete pipe inside a trench that was approximately 6 feet - 9 inches in depth on the south side at box #804 were exposed to crushing and struck-by hazard due to no trench protective system being used. (b) Employees engaged in installing concrete box #804 inside a trench that was approximately 8 feet - 6 inches in depth on the east side at box #804 were exposed to crushing and struck-by hazards due to no trench protective system being used. (c) Employees engaged in installing concrete pipe inside a trench that was approximately 9 feet - 7 inches in depth on the south side at box #802 were exposed to crushing and struck-by hazards due to no trench protective system being used. (d) Employees engaged in installing concrete pipe inside a trench that was approximately 7 feet - 3 inches in depth on the east side at box #802 were exposed to crushing and struck-by hazards due to no trench protective system being used.
