29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were not performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: (a) On or about December 28, 2018, at a work place located at 2990 TemTex Blvd, Terrell, TX; employees working from a man-made basket attached to the forks of a Toyota forklift, Model no. 8FGDU25, serial no. 74375, were exposed to a fall hazard.
