29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (a) Dispatch Office- Appendix D information had not been provided to the dispatch supervisor provided with and permitted to wear 3M 8247 model filtering facepiece respirators to protect against exposure to cremated ashes during their transfer from boxes to urns, on or about January 10, 2018. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
