29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and shall identify, evaluate, and control the hazards involved in the process. This violation was most recently observed on or about December 6, 2017 in the Di-benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas, when the employer did not identify, evaluate and control the hazards that are created when Hydrogen Peroxide relieves to an (unsafe location) inside the BPO manufacturing building (BLDG #16).
