29 CFR 1910.1026(d)(1): The employer did not determine the 8-hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI) in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section. On or about 3/8/18, and at times prior thereto, the employer allowed employees to conduct welding within the space of stainless steel intermediate bulk containers, utilizing stainless steel welding rods, and the employer did not conduct an exposure assessment of the atmospheric exposures of chromium VI.
