29 CFR 1910.147(f)(4): The employer did not ensure that the continuity of lockout or tagout protection, including provisions for the orderly transfer of lockout and tagout device protection between off-going and oncoming employees, to minimize exposure to the hazards of unexpected energization or start-up or the equipment or release of stored energy. On or about November 22, 2017, at the A&AT, LLC plant in La Porte, Texas, the employer did not ensure employees followed shift change procedures and communicated the status of energy control protections, specifically that the lockout protections on "C" Reactor had been modified.
