29 CFR 1910.303(b)(1)(viii): The employer did not ensure that electrical equipment was free from recognized hazards, based on the safeguarding of persons using or likely to come in contact with equipment: The employer is failing to protect employees from 480 volt electrical and arc flash hazards associated with working in the vicinity of unsecured enclosure doors. This was most recently documented on September 27, 2017 during the inspection of the facility located at 105 N. Main St. Wakefield, NE; where the employer has employees exposed to unsecured enclosure doors in electrical room 81. Electrical installations in the room have doors which are not secured allowing them to open freely which exposes personnel in the area to live parts and possible arc flash hazards.
