29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. On or about September 25, 2017 and prior to that time located at 11332 and 11334 Tamarco Dr. Blue Ash OH along interior and perimeter exterior edge areas of the structural steel structure employees performed metal decking dismantling and installation activities without conventional fall protection systems and while not maintaining structural stability of the metal decking at all times exposing employees to collapse and fall hazards in excess of 15 feet who were not trained on the applicable steel erection / dismantling hazards, associated fall hazards and applicable steel erection requirements contained in Subpart R.
