29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) At 2307 US Hwy 31S, Bay Minette, AL: On or about September 19, 2017, the employer exposed employees to struck-by and crushed-by hazards, in that employees were allowed to operate the JCB 930 forklift, Model 660802, that had fork attachments of unknown manufacturer. The attachments were not certified by the forklift manufacturer and did not have updated instruction plates, tags, or decals.
