29 CFR 1910.269(a)(2)(ii): Each qualified employee was not trained and competent in the skills and techniques necessary to distinguish exposed live parts from other parts of electrical equipment, skills and techniques necessary to determine the nominal voltage of exposed live parts, minimum approach distances was not specified in this section corresponding to the voltages to which the qualified employee will be exposed, and the proper use of the special precautionary techniques, personal protective equipment, insulating an shielding materials, and insulated tools for working on or near exposed energized parts of electric equipment: (a)Cherry-Todd Electric Cooperative, Inc. @ Pole #1476 located on Rosebud Cut-Across Road, Mission, SD 57555: On and before September 9, 2017, the employer did not ensure employees were trained and competent in the safety-related work practices to include minimum approach distances, special precautionary techniques, insulated tools and PPE. On September 9, 2017, a lineman came into contact with 14.4kV as a part of moving the existing conductors in preparation for a new electrical pole. The condition exposed employees to burn, shock, and electrocution hazards.
