29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Throughout facility: Personal locks or tags were not affixed by each employee to a group lockout device when working with other employees when maintenance or servicing was conducted on equipment, such as, but not limited to, Thermoform, Toshiba and Nissei presses, on or about 10/2/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
