29 CFR 1910.253(a)(3): Apparatus such as torches, regulators or pressure-reducing valves, acetylene generators, and manifolds used in an oxygen-fuel gas system had not been approved: Employees were exposed to fire and explosion hazards. In the maintenance area a portable oxy-fuel torch cart had an acetylene regulator whose gage was missing a protective cover and was ready for use.
